Catalyst (Full Service Partnership)

Catalyst is an Assertive Community Treatment (ACT) program. Catalyst provides full service partnership services to Transitional Age Youth (TAY) ages 16 to 25 with a serious mental illness, and who are homeless or at risk of becoming homeless in San Diego County. This recovery-focused program is also co-located with a consumer-run clubhouse offering a wide range of services designed to help each participant lead a meaning, self-sufficient life.

SAY San Diego- Family Self-Sufficiency Program

Family Self Sufficiency (FSS) coaches partner with unemployed or under-employed San Diegans looking to explore new personal and professional opportunities. Whether you need emergency assistance, help to increase your income, job training, further education, free tax preparation, or guidance finding a new job or career, SAY is here!

SLR- Student Loan Report

The Student Loan Report was started by Drew Cloud in 2015 to provide unbiased coverage on the latest student loan news and information.

The Student Loan Report also creates resources and guides for borrowers and their families so they make the best decisions when it comes to paying for college and repaying student debt.

Rady Children’s Hospital

Rady Children’s Hospital-San Diego is a nonprofit, 551-bed pediatric-care facility dedicated to excellence in care, research and teaching:

  • We are the only hospital in the San Diego area dedicated exclusively to pediatric healthcare and the region’s only designated pediatric trauma center.

NPI Lookup Federally Qualified Health Center – FQHC – SAN DIEGO, CA

With the implementation of the Affordable Care Act (“Obamacare”), the need to find healthcare providers by consumers as well as medical billers and coders has dramatically increased. One of the provisions of the Affordable Care Act requires all providers of medical or other items or services & suppliers that qualify for a National Provider Identifier (NPI) to include their NPI number on all applications to enroll in the Medicare and Medicaid programs and on all claims for payment submitted under the Medicare and Medicaid programs.

This Database list several FQHCs in San Diego County and all over the country. 

EatFresh

EatFresh is an online resource for CalFresh (SNAP, or food stamps) eligible individuals and families; though this website is a great resource for anyone who wants to improve their health. EatFresh’s goals are 1) to encourage cooking at home with fresh foods and minimally non perishables, 2) to show users that healthy change happens even though barriers exist, 3) to better understand the link between lifestyle/diet choices with the prevention of chronic diseases.

San Diego County Public Assistance Line

The Access Customer Service Center is a call center and is an extension of the County of San Diego Family Resource Centers (FRC). Serving residents, providers, and other government agencies with questions or needs related to Family Resource Center programs and services, including providing case management services for existing FRC customers countywide. Access customer service representatives are experienced eligibility workers who are trained in CalWORKs, CalFresh, and Medi-Cal programs. Access Agents complete eligibility determinations, take application requests, and update existing case information.

2nd Avenue Transitional Housing For Single Men, San Diego Rescue Mission

Transitional Housing for single men for up to 24 months. Clients must be earning a permanent taxable income and be willing to complete thirty hours per week of work, school, or other structured activity in the community. Clients are required to pay a bed fee of 30% of their income and to save 30% of their income on a monthly basis.

Clients also participate in intensive weekly case management.

Areta Crowell BPSR Center

Areta Crowell Wellness Recovery Center’s services are based on the principles of psychosocial rehabilitation, with emphasis on attending to the whole person and re-establishing one’s life as a result of the consequences of mental illness, rather than simply attending to the symptoms of the illness. The services focus on recovery, rehabilitation, and community integration, and are individualized to meet the needs of each client. Clients actively participate in setting service plan goals and are encouraged to involve family and significant others in the recovery plan.
